What: Submission of requirements for the recognition of student organizations
When: from October 5, 2020 to October 22, 2020
✅Requirements:
• List of Officers
• Calendar of Activities
• Official logo of the Organization
• Resume/CV of the Adviser (s)
• Latest Certificate of Grades of the Officers
• picture of the Officers with caption identifying the individual position
• Constitution-and By-Laws approved by the Officers and the Advisers
• Updated list of members
• Bio data of the Organization Officers
• Certificate of Good Moral of the Officers
• Acceptance Letter from the Adviser
✅For old Organization: (Additional Requirement)
• Accomplishment Report
• Financial Report
• Previous Plan of Activities
• Certificate of Audit
How to Submit:
1. Submit the soft copy of the documents via electronic mail: osasmain.studentdevelopment@cvsu.edu.ph
3. Submit your documents via dropboxes located at CvSU-Main Gate 1 and Gate 2
