ANNOUNCEMENT:
2nd Semester SY 2020-2021 Application for Admission for MAIN CAMPUS will start on December 17, 2020 to February 4, 2021.
Online submission of application and requirements –
The following are the details:
a. Applicant will go to this link ( https://registrar.cvsu.edu.ph/osas/ ) and answer the online form
b. Applicant will download the accomplished online form and attach 1×1 ID picture and put signature
c. The following are the required documents/credentials which can be submitted either in hard copy (thru mail, parcel, courier, drop box) or soft/scanned copy or screenshot (thru email, FB messenger, Google form link):
First year applicants:
- Copy of the filled-out application form for admission;
- Grade 12 report card; and
- Certification of good moral character
ALS passer applicants:
- Copy of the filled-out application form for admission;
- Certificate of Rating; and
- Certification of good moral character
Transferee applicants:
- Copy of the filled-out application form for admission;
- Certification Credentials – Transcript of Records/Certificate of Grades, Honorable Dismissal, Certificate of Good Moral Character
- NBI Clearance
Second-course applicants:
- Copy of the filled-out application form for admission;
- Certification Credentials – Transcript of Records/Certificate of Grades, Honorable Dismissal

e. The grades reflected in the Transcript of Record (TOR) / Certificate of Grades (COG) of transferees and second coursers will be evaluated in accordance with the admission/retention policy of CvSU. The grade requirement for transferee is GPA of 2.00 (equivalent of 85%) or better and no failed grades.
f. No transferee or shiftee shall be admitted for the following programs:
- Engineering programs – after the first-year level
- Doctor of Veterinary Medicine program – after the fourth-year level
- Other programs – after the second-year level
